Check HERE for the form to aid as a check list for what must accompany all application forms. This is the year that most of the 5-year PA background checks will be due. We will issue this receipt for each application that is submitted to the office. Please note that because of the immense number of items to be turned in that it is required that all items must be turned in with the application including payment.
Incident Reporting
Safety is the #1 priority for our scouts and scouters. If your unit has an incident, such as a minor or major first aid incident or a near miss (almost an incident that could have resulted in an injury), please fill out an Incident Report and submit to the Council Service Center. While it seem weird to report an incident that didn’t happen, the report states you were aware of the potential incident and how to change the behaviors and actions to make sure it doesn’t occur in the future.

Unit Fundraising?
If your unit is planning a fundraiser, remember to fill out the Unit Money-Earning Application and submit it to the Council Scout Executive a couple of weeks in advance of the start of your fundraiser. Units running a fundraising event that do not have an approved money-earning application will not be covered under BSA Insurance.

Camp Lavigne Alumni Association
Help support our youth so they can enjoy Camp Lavigne by joining the Camp Lavigne Alumni Association. Membership is July 1 to June 30 and dues are $100/year. Upon joining, members will receive a “members only” neckerchief and Council Shoulder Patch. 100% of your membership dues goes to the Camp Lavigne Campership Fund. Fill out the Membership Form and submit to the Council Service Center.
Eagle Scout Alumni Association
Eagle Scouts, are you ready to support the current youth in our Council? Join and 100% of your membership dues goes to the Membership Fund to offset membership fees for the youth in our Council. Membership is July 1 to June 30. Membership dues are:
— Youth Eagle Scouts 18 and under – Free
— Adult Eagle Scouts 19-30 – $25/year
— Adult Eagle Scouts over 30 – $50/year
Upon joining, members will receive a “members only” Council Shoulder Patch. Fill out the Membership Form and submit to the Council Service Center
Going Camping?
If you are going to a weekend camp or less than 4 days, you will need to complete the Medical Form A-B. You can download it Short-Term Medical Form.
If you are going to summer camp or any week-long camp, you will need to complete the Medical Form A-B-C. Part C is a physical completed by a doctor. You can download it 4+ Day Medical Form.
